Doctors can help manage chronic diseases to reduce symptoms, improve quality of life and reduce the risk of deterioration leading to unnecessary hospital attendances. Such chronic conditions include diabetes, congestive cardiac failure, HT etc.
Eligible patients may qualify for a chronic disease management plan designed to improve health outcomes, prevent complications, and provide personalized care tailored to each individual's needs. These plans include up to 5 Medicare-subsidized visits to allied health clinicians, such as podiatrists or physiotherapists.
